The forms are available in either MS Word format, or Adobe PDF format. Click on the desired link to download the form.
After downloading the entry form, you may either:
 
1)  Print the form and mail it to Peter Macler; or,
2)  Open the form in MS Word or Adobe PDF format and simply complete the form
      (you may need to press the "insert" key to keep the same layout).
      After you have completed the form - DON'T FORGET to type "I AGREE" and YOUR NAME
      (in place of your signature) in the signature line at the bottom of the form.
3)  Attach the completed form to an email and send it to Peter Macler.
